Hypertension, is it reversible?

Short answer: sometimes yes, sometimes no, and knowing which category you are in is the first honest step. This piece explains what determines the difference and what the reversal work actually looks like when reversal is on the table.

The split that matters.

Hypertension divides into two categories, and the reversal question hinges on which one you have. Primary hypertension is the version driven by lifestyle, weight, insulin resistance, sleep quality, alcohol intake, sodium sensitivity and cumulative stress. It has no single identifiable cause, and it accounts for about 90 to 95 percent of adult hypertension cases in the UK. Secondary hypertension is driven by a specific underlying condition (kidney disease, adrenal tumour, thyroid dysfunction, medication side effect) and represents the other 5 to 10 percent.

Primary hypertension is where the reversal conversation is real. Secondary hypertension needs its underlying cause identified and treated by an appropriate specialist. Getting this split right is the first job of any responsible practitioner, and it is why blood pressure medication should never be discontinued without medical supervision even when the reversal work is going well.

What the reversal work actually looks like.

When primary hypertension is on the table, the levers are the same metabolic ones that come up across the rest of the practice, adapted for cardiovascular focus. Reducing insulin resistance drops blood pressure directly, because chronic hyperinsulinaemia contributes to vascular stiffness and sodium retention. Losing visceral fat matters more than losing weight in general. Consistent aerobic and resistance work does more for blood pressure than either alone. Sleep quality shifts the numbers meaningfully. Stress load shifts the numbers meaningfully. And a small subset of people respond dramatically to sodium reduction while most respond marginally.

The clinical experience is that a client who commits to the protocol will typically see systolic pressure drop 10 to 20 mmHg over three to six months. The reduction is meaningful in medical terms and meaningful in day-to-day terms. It happens in coordination with the client's GP, so that any medication reduction is tracked and gradual.

When to keep taking your medication.

Always, unless a qualified doctor tells you otherwise. Blood pressure medication is one of the most important interventions in modern medicine, and stopping it prematurely because a wellness protocol felt promising is one of the most common ways this work goes wrong. The reversal question is not "medication or lifestyle." It is "how much lifestyle change is possible, and how far can that safely reduce medication under medical supervision over what timescale."
